Coaching Agreement


This agreement is between Relevant Insight LLC, (“Coach”) and (“Client”), whereby Coach agrees to provide Coaching Services for Client. The International Coaching Federation (ICF) defines coaching as follows, “Coaching is a partnership (defined as an alliance, not a legal business partnership) between the Coach and the Client in a thought-provoking and creative process that inspires the clients to maximize their personal and professional potential.”


1. I understand that coaching is a Professional-Client relationship designed: 1) to facilitate the creation/development of personal, professional, ministry, and/or business goals, and 2) to create and carry out a strategy/plan for achieving those goals.


2. I understand that coaching is a comprehensive process that may involve all areas of my life, including work, finances, health, relationships, education, and recreation. It is designed to address issues that I, the client, would like to consider. I acknowledge that deciding how to handle these issues, incorporate coaching into those areas, and implement my choices is exclusively my responsibility.


3. I understand that coaching can involve a number of methods including brainstorming, values clarification, written assignments, goal setting, identifying plans of action, accountability, making requests, agreements to change behavior, and questioning.


4. I understand that coaching is not therapy, counseling, advice-giving, mental health care, or treatment for substance abuse or mental disorders. My coach is not a licensed mental health professional, and coaching is not a replacement for counseling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment, recovery from past abuse, professional medical advice, financial assistance, legal counsel or other professional services. I will not use coaching in place of any form of diagnosis, treatment, or therapy.


5. If I am currently in therapy or otherwise under the care of a mental health professional, I have consulted with the mental health care provider regarding the advisability of working with a coach and that this person is aware of my decision to proceed with the coaching relationship.


6. I understand and agree that I am fully responsible for my physical, mental, and emotional well-being during my coaching calls, including my choices and decisions.


7. I agree to communicate honestly, be open to feedback and assistance, and create time and energy to participate fully in the program.


8. I understand that my information will be held as confidential unless I state otherwise, in writing, except as required by law.


9. I understand that certain topics may be anonymously and hypothetically shared with other coaching professionals for training or consultation purposes.


10. I understand my Coach may refer me to another coach, resource, or mental health professional when he deems it necessary or appropriate.


11. I agree to allow my coach to submit a confidential record of my name, phone number, and e-mail address to the International Coach Federation (ICF, coachfederation.org) to further meet the on- going credentialing requirements. I understand that the ICF will only use this information to verify my coach’s professional requirements, and that the ICF will only maintain this information in its secure files.


12. I am aware that my Coach or I can choose to discontinue coaching at any time with written notice.


13. I understand that some or all of my coaching may be done by phone, Skype (or similar method), or in person. I will make scheduled session times. I understand that rescheduling and cancellations need to be made at least 24 hours in advance. Rescheduling for a valid emergency is allowed, otherwise missed sessions are not made up or refunded.


14. I understand that this coaching program is purchased on a subscription / retainer basis, which includes one or two scheduled sessions, along with reasonable e-mails and short phone calls during the course of each month.


Limited Liability: Except as expressly provided in this agreement, the Coach makes no guarantees or warranties, express or implied. In no event will the Coach be liable to the Client for consequential or special damages. Notwithstanding any damages that the Client may incur, the Coach’s entire liability under this agreement, and the Client’s exclusive remedy, will be limited to the amount paid by the Client to the Coach under this agreement for all services rendered up until the termination date.


This is the entire agreement of the parties, and reflects a complete understanding of the parties with respect to the subject matter. This agreement supersedes all prior written and oral representations.
